CEU Application Details

The CEU Program Information form CEU Program Information form begins the approval process.

1. Please fill out the CEU Program Information form, obtain signatures from the program coordinator or course facilitator and an ASU faculty member or sponsor for your program. The program coordinator or course facilitator is responsible for organizing the administrative aspects of the program. Faculty member or sponsor must be an ASU faculty member or academic professional with the expertise necessary to review the program and validate the program quality to award CEUs through the ASU Online and Extended Campus. Please note that the program must be co-sponsored by Arizona State University, with involvement by our faculty. ASU is unable to support independent agency conferences. If the program is being initiated by an organization outside of the university, with no university faculty affiliation, contact ASU Online and Extended Campus for further information.


2. Return the CEU Program Information form with the necessary attachments, including the program description, agenda, brochure, and faculty member and instructor vitae. These attachments must accompany the CEU Program Information form. To insure ample processing time, this form should be received by our office at least one month prior to the program start date. The application process must be completed each time a program is produced, unless the program is one of a series or the same program participants will be attending every session.

There is no fee to apply for CEU program approval. Once your program has been reviewed and approved, we will ask that you fill out a CEU Completion Report at the end of your program. A $15 processing fee is charged for each participant choosing to receive CEUs. Checks must be made payable to Arizona State University. Upon receipt of the completion report and payments, the CEU certificates will be processed and delivered.
